DIAGNOSTIC DILEMMA:
This month’s diagnostic dilemma A 2˝ years old girl presents with progressive breathlessness, alternating constipation and diarrhea since 3 months and fever since 4-5 days. On examination she had Pallor, axillary lymphadenopathy, hepatosplenomegaly and cardiomegaly. Her investigations revealed anemia with lymphocytosis, military shadows on XRC and chronic hepatitis on liver biopsy. What is the differential diagnosis? HYPER IGM SYNDROME PRESENTING AS RECURRENT NEONATAL INFECTIONS:
Hyper IgM syndrome is a congenital immunodeficiency that is due to a predominant primary defect of antibody production. Patients have very low serum concentrations of IgG and IgA with normal or elevated concentration of polyclonal IgM.
