Last month’s query of a 6 month old child presenting with lice over her both eye lids had several replies from the viewers. Most viewers have suggested removeal manually after application of Vaseline as probable treatment guidelines.

The authors have replied to the diagnostic dilemma in November 2005 of ?Viral Encephalitis in which they have replied that they had similar 5 cases from the same area and all of them responded to antituberculous therapy.

A 5 year old boy presents with fever and generalized lymphadenopathy since 9 months. He is Mantoux positive and axillary lymphnode biopsy revealed reactive lymphadenitis. He is treated with Antituberculous drugs but does not have much improvement. Abdominal lymphnode biopsy reveals MDR TB. Why the axillary lymphnode biopsy did not pick up Tuberculosis? Query: A 7 year old girl presents with splenomegaly and anemia. Workup for hemolytic anemia, storage disorder, malignancy, portal hypertension and bone marrow suppression is negative. What is the cause of her splenomegaly?
Answer: With storage disorder, hemolytic anemia, portal hypertension having been ruled out most common causes have been ruled out. However, this child resides in an area endemic for malaria and hence one should rule out Tropical splenomegaly. Here a malaria IgM & IgG test would be required. In this child malaria IgM was positive. She was treated with chloroquine for 6 weeks following which her splenomegaly regressed. Discuss this case further in the section of “DISCUSSION GROUPS”.
