DIAGNOSTIC DILEMMA:
This month’s diagnostic dilemma: Dr Deepak A 2 years old girl presents with pancytopenia. A bone marrow shows hemophagocytosis. She develops hepatosplenomegaly and lymphadenopathy. Other work up for VAHS is negative. A repeat marrow shows blasts like cells with no megakaryocytes or myeloid cells. Lymphnode biopsy is inconclusive. She dies due to intracranial bleed. A 7 years old boy presents with scholastic backwardness since 5 months, headache since 10 days, projectile vomiting since 5 days and right sided focal convulsion with fever since 1 day. MRI brain shows large SOL in right occipital lobe, bilateral medial temporal lobes, right posterior temporal lobe and left occipital lobe anteriorly. What is the cause of the SOL? Last month’s teaching file: Query: An 11 year old girl presents with recurrent megaloblastic anemia that improves with oral Vitamin B12 and folic acid but recurs once again when the supplements are stopped. How should this child be treated?
Answer: Recurrent megaloblastic anemia in an older child could be due to poor nutritional diet or could be due to deficiency of Intrinsic factor. This child has NUTRITIONAL MEGALOBLASTIC ANEMIA. Thus apart from B12 supplementation she would require a dietary modification. Discuss this case further in the section of “DISCUSSION GROUPS”.
