DIAGNOSTIC DILEMMA:
This month’s diagnostic dilemma: Dr Madhulika, Pediatric Medicine, SPMCHI, Jaipur
An eleven month old girl presented with diarrhea, fever, loss of neck control and intermittent staring gaze. Serum biochemistries and CSF examination were normal. Neuroimaging showed bilateral basal ganglia calcifications with symmetric hyperintensities in bilateral globus pallidus and putamen. Patients head lag persists.

Last month’s queryof a 10 month old child with fever, anemia and hepatosplenomegaly with normal hemoglobin electrophoresis, bone marrow aspiration and negative serology for viral markers had several replies. Most viewers have suggested hemolytic anemia and storage disorder as probable causes. The authors have replied that the child is undergoing a splenectomy at the moment.

This month’s teaching File: A 35 days old boy presented with cyanosis and jaundice since day 5 of life. He was diagnosed to have Ventricular Septal Defect with Pulmonary Atresia. Query: A 4 month old presented with neonatal cholestasis and negative TORCH titres with normal excretion on MRCP, normal thyroid function tests and liver biopsy showed cholestasis with multinucleation. What is the etiology of neonatal cholestasis?
Answer: This child has a neonatal cholestasis with yellow colored stools. Thus obstructive etiology seems unlikely. An infective etiology seems unlikely as he does not have any evidence of other system involvement which would be seen usually with a congenital infection. Thus, one would consider a metabolic cause such as galactosemia, fructosemia, tyrosinemia, Alpha-1 antitrypsin deficiency and even cystic fibrosis. Investigations revealed galactosemia .
