DIAGNOSTIC DILEMMA

A 10 years old boy born of third degree consanguineous marriage presented with gradual onset weakness of lower limbs since 2 years. He had a generalized tonic convulsion at 5 years of age and then repeat convulsions every 15 to 30 days till 7 years of age. He is convulsion free since last 3 years. He was given MDI with Budesonide and Salmeterol and gradually his tachypnea decreased. He was suspected to have Bronchiolitis obliterans post pneumonia.

A 4 year old presented with splenomegaly, hematemesis and malena. Colour Doppler did not show any collaterals initially but repeat colour doppler by the same sonologist after 5 days showed presence of portal cavernoma. Why did the earlier colour doppler not pick up portal cavernoma? Read the entire teaching clinical query in the section of “TEACHING FILES” and try the correct answer.

Query: An 8 month old boy presented with multiple episodes of generalized tonic clonic convulsions On examination he had right sided hemiplegia without any cranial nerve involvement. What is the cause of the hemiplegia and where is the CNS lesion?

Answer:Right temporal infarct and workup for thromboembolic phenomenon revealed Protein S deficiency.
