Last month’s queryof a 3-years old boy with Fever of unknown origin, diarrhea and vomiting since 25 days, thrombophlebitis and ARDS with negative cultures had suggestions from the viewers that included HIV, Immunodeficiency, Leucocyte adhesion defect, Protein losing enteropathy, inflammatory bowel disease. The authors have stated that they ruled out all possibilities except LAD and Inflammatory bowel disease. They treated this child as a case of Atypical Kawasaki in view of prolonged fever, multisystem involvement, phlebitis, epididymitis, thrombocytosis and temporary relief on Immunoglobulin and started on Prednisolone and Aspirin following which the condition improved dramatically.

Investigations reveal neutrophilia with thrombocytosis, normal PTH levels and low immunoglobulins. What is the diagnosis? Last month’s teaching file:
Query: A 6 years old girl with UTI and micturating cystourethrogram (MCU) suggestive of Grade II reflux on the right side and Grade III reflux on the left side with DMSA scan showing no renal scars. Does she require surgical intervention for her VUR?
Answer:The child requires only conservative line of management and prophylactic antibiotics to prevent breakthrough UTI. Surgery at the moment is not required. Discuss this case further in the section of “DISCUSSION GROUPS”.
