DIAGNOSTIC DILEMMA:
This month’s diagnostic dilemma:Dr Shah
A preterm develops neonatal meningitis followed by chronic ventriculitis and hydrocephalus. He is treated with antibiotics for 6 weeks, antifungals for 3 weeks but the CSF still shows low sugar. He undergoes repeated ventricular taps for his hydrocephalus as chamber/shunt is contraindicated due to high CSF proteins. How to treat this child further?

Query: A 5 year old boy with fever and generalized lymphadenopathy and Mantoux positive had axillary lymphnode biopsy revealing reactive lymphadenitis. He is treated with Antituberculous drugs but does not have much improvement. Abdominal lymphnode biopsy reveals MDR TB. Why the axillary lymphnode biopsy did not pick up Tuberculosis?
Answer: When a node biopsy is indicated, excisional biopsy of the most abnormal node will best enable the pathologist to determine a diagnosis. Since the most abnormal lymphnodes were in the abdomen, abdominal lymph node biopsy is most specific and sensitive and a peripheral lymph node biopsy may not lead to a proper diagnosis as was seen in this case.
