DIAGNOSTIC DILEMMA:
This month’s diagnostic dilemma: A 3-years old boy has Fever of unknown origin, diarrhea and vomiting since 25 days. He has also developed thrombophlebitis and ARDS but cultures are negative. Investigations reveal persistent Leucocytosis, high CRP, high ESR, thrombocytosis, negative Widal test, ANF, Anti-ds DNA, Rheumatic Factor, LE cells, ANCA levels, normal immunoglobulins, normal NBT test. Last month’s queryof a 6-year-old boy with ascitis, generalized edema, hypertension and hypoalbuminemia with normal renal function tests had various replies ranging from polycystic kidney disease to protein losing enteropathy. The authors have replied that kidney disease had been ruled out and are now working up the child on the lines of protein losing enteropathy as well as Tuberculous peritonitis. Query: An 8 year old with recurrent otitis media and past history of leukemia. What is the cause of her recurrent otitis media?
Answer:One vital examination finding in this patient that had been missed was taking blood pressure. As we all know that hypertension may just present as headache. In this child, on examination, his BP was 240/160 mm of Hg. Cause of headache was hypertension. Unfortunately, only one viewer has answered this query correctly.
