DIAGNOSTIC DILEMMA: Last month’s query of a 1½-year-old boy with recurrent hematuria, left sided epididymitis and splenomegaly and kidney biopsy suggestive of mild proliferative glomerulonephritis received timely answers from the experts. One suggestion of a Familial Mediterranean Fever was very valuable and the child is now being investigated from that angle.

This month’s problem case is of:
An HIV positive infant with perinatal transmission of HIV and chronic hepatitis. The mother had received antenatal zidovudine and the baby also received zidovudine at birth. Antiretroviral therapy and antituberculous drugs were also given in the child. Chronic hepatitis is unexplained. Is it HIV induced hepatitis or Antenatal exposure to zidovudine and subsequent liver damage or drug induced hepatitis due to ART and AKT.

Last month’s teaching file:
A 4-½ years old male presents with bull neck and proteinuria and is treated with IV albumin. Is the therapy correct and what should be the further management?

Answer: The therapy is wrong and every proteinuria does not require treatment with IV albumin. Since the history is suggestive of diphtheria, one should rule out glomerulonephritis and myocarditis.

REYE’S SYNDROME – AN INBORN ERROR OF METABOLISM:
Reye’s syndrome is characterized by acute encephalopathy and fatty degeneration of the liver. An increasing number of inherited mitochondrial metabolic diseases have been described that produce episodes like Reye’s syndrome. This is a case report of a child who presented with Reye’s syndrome due to an inborn error of metabolism.

CONGENITAL HYPOTHYROIDISM:
The prevalence of congenital hypothyroidism approximates 1 in 4000 birth. Delay in diagnosis may lead to mental delay and developmental delay. We highlight this case that compulsory screening of all newborns for hypothyroidism would have helped in the diagnosis of this child much earlier and treatment could have been started early.
