Last month’s query of a 7-year-old boy with persistent fever, leucocytosis, pericardial effusion and joint pains since one month had several replies and most of the viewers had suggested it to be systemic onset JRA. The authors have also replied that they have treated the child as Systemic onset JRA as with evolution of the disease, he had involvement of T-M joint and also cervical spine. The child has been started on Methotrexate and steroids have been reintroduced but the child continues to have fever.

Query: A 7 years old boy presents with recurrent hypocalcemic convulsions and fever. On examination, he has generalized lymphadenopathy. Investigations reveal neutrophilia with thrombocytosis, normal PTH levels and low immunoglobulins. What is the diagnosis?
Answer:The diagnosis in this setting would be "COMMON VARIABLE IMMUNODEFICIENCY". Common variable immunodeficiency (CVID) is the most prevalent of the primary immunodeficiency diseases. Discuss this case further in the section of “DISCUSSION GROUPS”.
