Last month’s teaching file: Query: A six-month-old presents with severe metabolic acidosis, polyuria and polydipsia with neuroregression. What is the diagnosis?
Answer: The diagnosis “LACTIC ACIDOSIS DUE TO INBORN ERROR OF METABOLISM”. This child has a positive anion gap metabolic acidosis with neuroregression. One cannot rely on history of polyuria and polydipsia in a 6 month old without objectively measuring the 24 hours urine output. Discuss this case further in the section of “DISCUSSION GROUPS”.
