She had fever with bilateral knee joint pain 7 months back and also a history of a photosensitive rash over face on exposure to sun. What is the diagnosis? Query: A 7 years old boy presents with scholastic backwardness since 5 months, headache since 10 days, projectile vomiting since 5 days and right sided focal convulsion with fever since 1 day. MRI brain shows large SOL in right occipital lobe, bilateral medial temporal lobes, right posterior temporal lobe and left occipital lobe anteriorly. What is the cause of the SOL?
Answer: Tuberculomas. One may consider brain tumor but all the workup for malignancy is negative. In children, tuberculomas are infratentorial, located at the base of the brain near the cerebellum, often singular but may be multiple. Discuss this case further in the section of “DISCUSSION GROUPS”.
