DIAGNOSTIC DILEMMA

9 years boy presents with edema, breathlessness, headache, gallop rhythm, raised JVP, hypertension and tender hepatomegaly. Echo revealed ASD, USG-abdomen revealed non visualization of right kidney. What is the probable diagnosis and further treatment?

Vivek Chetal. There have been studies on ranitidine and PPI's causing community acquired pneumonia in children using them for GERD or acid peptic diseases. My question is what other option do we have if not gastric acid inhibitors? And how ominous is the threat?

Indira Nagesh Shetty. A neonate has severe scabies. How to treat?

Nishant: A 14 years old female child with history of fever (recurrent episodes) for last 8 months and ?constitutional growth retardation (weighs only 25 kg) with no secondary sexual characters. She has been investigated in CMC Vellore. All reports including bone marrow and karyotyping are normal. Only positive finding is that urine culture grew E.coli. Presently child is on IV antibiotics but main problem for last 20 days is persistent retching and vomiting not responding to any modality. Barium studies, endoscopy, USG are normal. She has received treatment for h.pylori also. Any suggestions for future course.
