Five year old boy presented with fever, petechiae, gangrene at ear pinna, convulsions (once) and had signs of raised ICT, malena, tachycardia. CSF: proteins 433 mg/dl, sugar 82, cells-10 (100% lymphocytes). Being treated as viral encephalitis. What is the diagnosis? Read the case further in detail…

Last month’s queryof a 1 year-old girl with facial puffiness and polyserositis had several replies suggesting protein losing enteropathy, SLE and lymphatic obstruction. However, the authors have replied that the child Pleural tap was repeated after 6 weeks of antibiotics and the report is 140 cells with 100% lymphocytes with 3.4gm of proteins. Serum proteins are 3.0 gm/dl. The child does not have any diarrhea for protein losing enteropathy. Ether test on the pleural fluid was negative ruling out chylous fluid. Workup for SLE was normal. Thus the child has been started on AKT. Also the child had hypertension and was treated with antihypertensives. Workup for hypertension: USG kidneys and colour doppler of renal arteries: normal, S cortisol normal, MIBG and VMA normal. Echocardiography showed mild pericardial effusion.
