DIAGNOSTIC DILEMMA:
This month’s diagnostic dilemma: Dr Naji
A consanguineous married couple has 6 children, 3 of whom have died due to infections and one 3 year old girl currently is suffering from recurrent lung infections, clubbing, FTT and generalized lymphadenopathy. Workup for immunoglobulins, cystic fibrosis is negative. Skin lesions are suggestive of TB. Lymph node biopsy is suggestive of follicular hyperplastic lymphadenitis. What is the diagnosis? This month's teaching File: A 13 years old presents with progressive tremors, poor scholastic performance, repeated episodes of falls with loss of consciousness, visual hallucinations, diplopia and slurred speech. A neurological opinion taken was suggestive of schizophrenia. Is the diagnosis of schizophrenia correct? Query: A 3 months old boy presents with bilateral cataracts. His TORCH, Urine Aminoacidogram, Serum calcium and blood gases are normal. What is the diagnosis?
Answer: Prematurity, intrauterine infections, hyperparathyroidism, syndromic variety, Lowe's syndrome are ruled out. Galactosemia is still a possibility especially Type 2 Galactosemia which may just present with cataracts and no hepatic or systemic manifestations. In this child Galactokinase deficiency was proved by enzyme estimation. Discuss this case further in the section of “DISCUSSION GROUPS”.

CASE REPORTS:
Cranio-Cerebral Erosion with Parafalcine CSF Collection: A Case Report.
Karam Chand, Hukum Singh, Vikas Gupta. An 18 months old child, presented with growing skull fracture. CT scan brain with 3D reconstruction showed Leptomeningeal cyst with skull defect. There was a large associated Parafalcine CSF collection on the side of herniating brain; the child was managed accordingly with good outcome.

Lamellar Ichthyosis in an Infant.
Rajniti Prasad, OP Mishra, U K Singh. Read about a case of lamellar ichthyosis in one year old child who presented with thick dark scaly lesions all over body with ectropion of both eyes and was treated with emollient and topical retinoid therapy.
